Default timing of command group designations

When setting up for BBDBA, when does a player need to designate the high-pip, mid-pip and low-pip commands, and when does he need to tell his opponent which command is which? Also, Do you ever need to disclose to your opponent which command is the C-in-C?

The PIP dice are designated when the commands are created, before deployment starts. You tell your opponent the first time you roll for PIPs. You disclose your CinC when his command is placed on the table.

By the way, in tournaments, you can usually shuffle elements between commands between games, but you can't change your army's overall composition.

That's exactly the way we do it here.
1) We first note down the exact composition of each command, and also note which is high/mid/low PIP command and which elements are generals and who is CinC.
2) On deployment we reveal the generals and CinC.
3) When rolling for PIPs the first time, we reveal high/mid/low PIP command.
